Are you using an Action Replay? I did that once; I took my Action Replay, enabled all cheats, and when I went to fight wild Pokemon, that question mark would pop up. You can instantly catch them with any ball, but don't bother; when you catch them, they turn into Bad Eggs, and Bad Eggs can't be hatched.
